Changes in Matriculation

To Change or Add a Subject

  • Get a changing or adding form from the Office of the University Registrar (OUR)
  • Fill out the form
  • Let the instructors sign and the Dean approve the request
  • Pay the changing or adding fee of Php 75.00 at the Cashier
  • Submit the approved form at the OUR

Note: Refer to Academic Calendar for schedule of adding or changing of subject

To Drop a Subject

  • Get a dropping form from the OUR
  • Fill out the form
  • Let the instructor/ssign and the Dean approve the request
  • Pay the dropping fee of Php 75.00 at the Cashier
  • Submit the form at the OUR

Note: Deadline for dropping of subjects is one day before Midterm exams
